The Role of Combined Synthetic Surfactant and CO2 on Sleep Apnea in a Brown Norway Rat Model of Asthma

Abstract Rationale Sleep apnea is a breathing disorder characterized by cessation or shallow, irregular breathing during sleep. Globally, over 100 million people are affected by sleep apnea, with up to 80% of moderate and severe OSA cases going undiagnosed. OSA results from the collapse of the upper airway during sleep, causing partial or complete obstruction and reducing or stopping airflow. Asthma and OSA often coexist, with OSA being more common in asthmatics and associated with asthma severity and control. Brown Norway rats (BN) can serve as a model for studying sleep apnea, with strains exhibiting varying levels of spontaneous apneas.Nebulized surfactant (Perflubron) combined with Inhaled CO2 combinedhas shown promise in dilating airways and reducing lung resistance in animal models of chronic allergic asthma, suggesting a potential new approach for treating sleep apnea. Method BN rats were sensitized to ovalbumin (OVA), receiving 1mL intraperitoneal injection of a solution containing 100μL OVA, 1.5g Al(OH)3, and 10μL Bordetella pertussis toxin in 10mL of 0.9%NaCl. On the 21st, 28th, and 35th days post-sensitization, rats were exposed to nebulized OVA for 10 minutes in a 15 L chamber. After exposure, rats stayed in the chamber for 15 minutes before transferral to a whole-body plethysmograph for pulmonary function testing. All rats had a minimum one-week washout period between OVA challenges. Different treatments were administered 1 week after the sensitization/washout period. The study received ethics approval, protocol MO9101. Results The response to nebulized Perflubron combined with 8% CO2 was remarkably rapid, commencing immediately upon administration of the treatment, and persisting throughout the 10-minute treatment period and for 5 minutes post treatment. Both the number and duration of apneas in animals treated with Perflubron-8% CO2 exhibited a significant improvement from those treated with other treatment combinations and medical air.Additionally, there was no difference between in response between delivery methods (ie. Vapour vs Nebulized). Conclusion The current study demonstrated that nebulized Perflubron-8%-CO2 is a potent formulation for reducing apneic events in both number and duration following OVA allergen challenge in the BN rat model of allergic asthma. However, it appears that this treatment effect is relatively short-lived, except at the higher doses. The mechanism of response is based on the properties of perflubron/CO2 on lung function as perflubron has mucolytics and surfactant properties and is an excellent carrier of respiratory gases.
