ADVERSE CHANGES IN SOCIAL TIES AND RISK OF HYPERTENSION: A PROSPECTIVE, GENDER-SENSITIVE STUDY OF THE CANADIAN LONGITUDINAL STUDY ON AGING (2011-2021)

Objective: To add to limited longitudinal literature on social ties and hypertension by prospectively examining alterations in multiple social ties and new-onset hypertension in aging women and men in Canada. Design and method: A population-based, prospective study of middle-age and older adults using 3 waves of Canadian Longitudinal Study on Aging data (CLSA, 2011-2021). We assessed unique transitions in marital status, living arrangements, and social isolation (wave 1 to 2) and new-onset hypertension at wave 3. We performed 10 imputations and ran multivariable logistic regression, stratified by (cis-)gender (n=13,773 women; n=13,086 men). Results: Preliminary results showed that specific adverse changes in all three social connections increased the odds of new-onset hypertension among women only. Women who remained socially isolated between had 91% higher odds of developing hypertension at (OR 1.913 [1.25, 2.93]), compared to women remaining not socially isolated. Women who remained lone-living had 38% higher odds of developing hypertension (OR 1.38 [1.14, 1.67]), compared to those remaining co-living. Women who remained non-partnered or who became widowed had 32% or twofold higher odds of developing hypertension (respectively, 1.32 [1.12, 1.56] and 2.02 [1.18, 3.43]) compared to remaining partnered. Results for men were non-significant, although a few transitions in social connections (multiple/uncertain marital transitions, became lone-living, remain socially isolated) had higher odds ratios of new-onset hypertension at follow-up (range: 1.21 to 1.38). Future models will include key confounders (such as women’s reproductive status) and assess whether associations each social tie are independent of other social ties. We expect adverse changes (either persistent lack of social tie or loss of social tie) will result in developing hypertension particularly among aging women. Conclusions: This prospective study provide novel disaggregated data on alterations in three different social ties as potential determinants of new-onset hypertension among women and men in Canada. In particular, we assess marital transitions, changes in living arrangement and alterations in social isolation over three years to show specific alterations over time may lead to the development of hypertension only among aging women in Canada.
