POS1107 TIMING OF MATERNAL DEXAMETHASONE AND PREGNANCY OUTCOMES IN ANTI-SSA/Ro POSITIVE PREGNANCIES

Background: Maternal administration of dexamethasone is believed to improve outcomes in pregnancies complicated by congenital heart block (CHB) associated with anti-SSA/Ro antibodies. However, it is not known if the timing of dexamethasone administration may influence its efficacy. Objectives: We conducted a systematic literature review to investigate the relationship between the timing (week of gestation) of maternal dexamethasone administration and neonatal outcomes in anti-SSA/Ro-positive pregnancies complicated by congenital heart block. Methods: The review was based on the 2020 PRISMA guidelines. Databases searched were PubMed, OVID, Medline and Web of Science (from 1991 to present) with terms "dexamethasone," "neonat*", "death", "admin*" and "pregnan*", "anti-Ro". Data was collected in a pre-established collection form. Cases were categorized into two groups: <24 weeks (Group A) and ≥24 weeks of gestation (Group B, 32 cases). Primary outcomes included neonatal survival rates (1-year follow-up), pacemaker requirements and resolution of hydrops. Descriptive statistics were used. Results: We retrieved 12 studies (out of 204 screened) reporting on 80 cases. Dexamethasone was administered orally at doses of 4-12 mg/day until delivery. Group A (<24 weeks, n=48) had a higher survival rate (89.6%) compared to Group B (≥24 weeks, n=32, 84.4%), although not statistically significant. Pacemaker requirements were significantly lower in Group A (45.8% vs. 78.1%, p=0.004). There was no significant difference in the survival rates in cases with hydrops in Group A and B (p = 0.74). Conclusion: Early initiation of dexamethasone (<24 weeks) in anti-SSA/Ro-positive pregnancies improves neonatal outcomes by reducing pacemaker dependency. There were no significant differences regarding survival at 1 year and hydrops. These findings highlight the need for early monitoring and proactive management of at-risk pregnancies. Future strategies and studies should focus on enhanced screening protocols, early fetal monitoring, and the use of biomarkers suggestive of heart block to overcome barriers to timely intervention.
