P-415 Prevalence of Polycystic Ovary Syndrome Prevalence in Patients with Recurrent Early Pregnancy Loss: a Canadian Population Study

Abstract Study question This study aims to determine the prevalence of Polycystic Ovary Syndrome (PCOS) in patients with Recurrent Early Pregnancy Loss (REPL) in a Canadian population. Summary answer Among the 1000 women with REPL, 245 (24.5%) met the diagnostic criteria for PCOS. What is known already REPL presents a significant challenge, impacting approximately 2-5% of couples attempting to conceive. It is defined as the loss of two or more pregnancies before 12 weeks of gestation. Over the years, research has explored the relationship between REPL and PCOS, a complex endocrine disorder prevalent among women of reproductive age. Numerous studies have attempted to understand this relationship. Study design, size, duration This retrospective cohort study includes patients treated in a culturally diverse, private fertility practice in Toronto, Ontario, Canada. A thorough review of 1000 patient charts meeting the REPL criteria from 2008 to the present was conducted. Participants/materials, setting, methods PCOS diagnosis was determined based on the Rotterdam criteria, which requires two of the following: polycystic ovarian morphology or ovarian volume ≥10 cc on ultrasound, oligo- or anovulation, and clinical or biochemical hyperandrogenism. Demographic and clinical characteristics were analyzed as potential confounding factors. In addition, in a subset of the population diagnosed with REPL and PCOS, metformin was prescribed, and reproductive outcomes were followed. Main results and the role of chance Among the 1000 women studied, 245 (24.5%) met the diagnostic criteria for PCOS. Those diagnosed with PCOS were significantly younger (p < .001), with a mean age of 33.5 ± 4.1 years, and had a higher BMI of 27.1 ± 6.1 (overweight), compared to the non-PCOS group with a mean age of 35.3 ± 4.6 years and a BMI of 24.7 ± 4.5 (Normal weight) (p < .001), respectively. Fasting insulin levels were also found to be higher in the PCOS group. As well, a significant positive correlation between PCOS and number of losses was observed with a 3.83% higher risk of pregnancy loss in the PCOS group (p = .007). Furthermore, 63 of the 245 patients with PCOS were prescribed Metformin as empiric treatment. The rate of clinical pregnancy based on first ultrasound was statistically significantly higher in patients with PCOS who received Metformin (n = 53; 85.3%), as compared to those who did not receive the treatment (n = 29, 80.3%) (p < .01). Limitations, reasons for caution Due to the retrospective nature of this data collection, it was found that some data was missing. Another limitation is the subjectivity when assigning the Rotterdam criteria. Wider implications of the findings The results of this investigation suggest that the prevalence of PCOS in a population of women with REPL is higher than what has been historically suggested. There is also a need for further research into the role of Metformin in women with PCOS to reduce the rates of pregnancy loss. Trial registration number No
