Stroke profile of patients with high risk of obstructive sleep apnea: a transversal study

Obstructive sleep apnea (OSA) is a disease of upper airways, diagnosed with polisomnography, that is well associated with stroke throught diverse mechanisms. Despite that, it still lacks studies about stroke profile in patients with high risk of OSA. This study aims to describe the diferences between stroke in patients with high and low risk of OSA. We performed a transversal study, part of a cohort of stroke patients. Patients were evaluated of risk of OSA with STOP-Bang and SOS scores; also, data of wake-up stroke, stroke type and etiology, transient ischemic attack (TIA), thrombolysis, circulation evolved, ASPECTS (Alberta stroke program early computed tomography score), PC-ASPECTS (posterior circulation - acute stroke prognosis early computed tomography score), hemorragic transformation and its type (ECASS), ABCD2, National Institute of Health Stroke Scale (NIHSS) and death during internment were colectted. We obtained 430 patients, 50.5% female. 69% were classified as high risk with STOP-Bang, median 3 (interquartile range 2-4) and 50% with SOS score, median 10 (interquartile range 7-16). Considering STOP-Bang, there were significant diferences with previous stroke - most prevalent in high risk - (p=0.01) and admitional dyastolic pressure (p<0.001). Considering SOS, we obtained admitional capillary blood glucose (p=0.005), admitional dyastolic pressure (p=0.03). ASPECTS > 7 (p=0.02). In both scores, high risk patients evolved more to death during internment (p=0.04 and p=0.01, respectively). In baseline characteristics of STOP-Bang, there was a higher prevalence in high risk group of male sex (p<0.001), alcoholism (p=0.01), obesity (p=0.01), hypertension (p<0.001) and diabetes (p=0.002); of SOS score, female sex (p=0.007), obesity (p<0.001), hypertension (p=0.001) and diabetes (p=0.001). The stroke profile of patients with high risk of obstructive sleep apnea appears to follow the pattern of stroke in general: a higher prevalence of ischemic type and similar rates of hemorragic transformation. Otherwise, high risk of OSA patients have more recurrence of cerebrovascular event and tend to be admitted with higher measures of capillary blood glucose, dyastolic pressure and worst images, which is concordant with the information of higher prevalence of hypertension and diabetes. This phenomenon also can be explained due to metabolic pattern of this diasease, in addition to vasoreactivity impairment, which is demonstrated most with dyastolic measures.
