Stucture of Level I Perinatal Centers in Germany – can we learn from Canada?

Introduction Perinatal Centers play a crucial role in providing specialized care to high-risk pregnancies and newborns. In Germany, perinatal centers are classified into three levels based on their capacity to provide specialized care. Level III centers provide basic care, while Level I centers offer the highest level of care. In most other countries, including Canada, this classification is the opposite, with Level III centres providing the highest level of specialised care. We aim to explore the structure of Level I perinatal centers in Germany and compare it with the Canadian model to identify potential areas for improvement. Materials and Methods We present a comparative analysis of the structure of specialised high care perinatal centers in Germany and Canada based on the personal experience of the first author: experience of the German system through basic (Obstetrics & Gynaecology) and specialised (Perinatal Medicine) training over 8 years in Germany followed by 2 years of Clinical Fellowship (Maternal Fetal Medicine) in one of the largest tertiary Perinatal Centers in Canada. This analysis focuses on the organizational structure, staffing, training, and services provided by high care Perinatal Centers in both countries. Results Both Germany and Canada have a tiered system of perinatal care, with Level I Centers in Germany and Level III Centers in Canada providing the highest level of care. However, the Canadian model emphasizes regionalization of care, with a smaller number of high-volume centers, while the German model focuses on decentralization, with a larger number of lower-volume centers. In the Canadian system, the medical doctor has mostly a consulting, supervising role of various highly specialised trained medical professionals (various levels of nursing, sonographers etc) who fulfil a large part of the clinical work, whereas in Germany the medical doctors are mostly solely responsible for all clinical tasks. The Canadian model also places greater emphasis on multidisciplinary teams, standardized protocols and structured training. Discussion The Canadian model may offer some advantages over the German model in terms of efficiency, allocation of resources and quality of high end specialised care. However, the German model may be better suited to the country's decentralized healthcare system and regional variations in population density. In view of the upcoming hospital reform in Germany, it would be important to try to gain insight from the medical systems of other developed countries.
