Застосування шейверних технологій дозволяє виконати прецизійну корекцію трубного валика одночасно із виконанням аденотомії. Метою дослідження було вивчити клінічну ефективність шейверної ендоскопічної аденотомії з додатковою корекцією трубного мигдалика при наявності його гіпертрофії, за результатами опитування у віддаленому післяопераційному періоді. Для цього проведено опитування 209 пацієнтів, яким виконувалась ендоскопічна шейверна аденотомія. У 107 із них при наявності гіпертрофії трубного валика виконувалась його корекція (основна група), у 102 – лише аденотомія (група контролю). Вивчена поширеність основних скарг у пацієнтів перед аденотомією та їх динаміка за результатами опитування через 12–24 місяці після втручання. Виявлено, що додаткове видалення гіпертрофованих трубних валиків у дітей під час шейверної ендоскопічної аденотомії дозволяє достовірно знизити ризик частих запальних захворюваннях верхніх дихальних шляхів та гострих середніх отитів. Частка осіб, яка оцінила ефект аденотомії при опитуванні як «суттєве покращення» за критерієм «часті запальні захворювання верхніх дихальних шляхів» у основній групі була на 20,4 % більшою, а «часті гострі середні отити» – на 41,3 %. В результаті дослідження ми дійшли висновків, що ендоскопічна шейверна аденотомія із корекцією трубних валиків – ефективний та безпечний метод лікування дітей з гіпертрофією глоткового мигдалика у комбінації з гіпертрофією трубних валиків, особливо у випадках, коли показаннями до втручання є часті запальні захворювання верхніх дихальних шляхів та гострі середні отити. Archived: https://doi.org/10.5281/zenodo.15489104 Abstact In patients with hypertrophy of the pharyngeal tonsil, hypertrophy of the lymphoid tissue of the torus tubarius is often observed. The application of power-assisted technologies allows for precise correction of the torus tubarius simultaneously with adenoidectomy. The purpose of the study was to investigate the clinical effectiveness of power-assisted endoscopic adenoidectomy with additional correction of the torus tubarius in the presence of its hypertrophy, based on postoperative survey results. For this purpose, we surveyed of 209 patients who underwent endoscopic power-assisted adenoidectomy. In 107 of them additional correction of torus tubarius hypertrophy was performed in case of its presence (main group), while in 102, only adenoidectomy was performed in all patients (control group). The prevalence of main group complaints before adenoidectomy and their dynamics were assessed based on a survey 12–24 months post-intervention. It was found that the additional removal of hypertrophied torus tuba­rius in children during power-assisted endoscopic adenoidectomy significantly reduces the risk of frequent upper respiratory tract infections and acute otitis media. The proportion of individuals who assessed the adenoidectomy effect as "significant improvement" in terms of "frequent upper respiratory tract infections" in the main group was 20.4% higher, and for "frequent acute otitis media" it was 41.3% higher. Conclusion: Endoscopic power-assisted adenoidectomy with torus tubarius correction is an effective and safe method for children with hypertrophy of the pharyngeal tonsil in combination with torus tubarius hypertrophy, especially in cases where indications for intervention include frequent upper respiratory tract infections and acute otitis media.
