Detailed Description This project is a systematic review of reconstructive and cosmetic surgical techniques of the female genital, urethral, and anal tract, conducted according to PRISMA guidelines. Pelvic floor dysfunctions, congenital anomalies, obstetric trauma, oncological resections, and other acquired defects have profound consequences on urinary and fecal continence, sexual function, and overall quality of life. Reconstructive and cosmetic surgery offers a wide range of techniques—ranging from local and regional flaps, to microsurgical free flaps, to prosthetic devices and cosmetic procedures—that aim to restore both anatomy and function while also addressing aesthetic concerns. The purpose of this review is to systematically collect, appraise, and synthesize the available evidence published between 2000 and 2025 on reconstructive and cosmetic surgical interventions in women. The study will evaluate: Functional outcomes such as continence, fistula closure, and anatomical restoration. Patient-centered outcomes, including sexual function, quality of life, and satisfaction. Surgical safety outcomes, including complications, wound healing, and long-term sequelae. A systematic search of PubMed, Embase, and the Cochrane Library was performed up to May 31, 2025, including clinical studies (randomized controlled trials, observational studies, case series, and systematic reviews) in English and Italian. Risk of bias will be assessed using the Cochrane RoB tool for randomized studies and the Newcastle–Ottawa Scale for observational studies. The expected outcomes of this project are: A transparent overview of the reconstructive options available for different indications (oncologic, obstetric, traumatic, cosmetic). Identification of the most reliable techniques and their complication profiles. A critical appraisal of the evidence gaps, particularly regarding long-term outcomes, sexual function, and patient-reported quality of life. Practical recommendations for clinicians and future research directions, with emphasis on multidisciplinary management involving gynecology, plastic surgery, physiotherapy, and psychological support. By providing a rigorous synthesis of the literature, this review aims to inform clinical practice, guide surgical decision-making, and highlight areas where further high-quality research is needed.
