etexilate is an oral directthrombin inhibitor approved for the prevention of thromboembolism in atrial fibrillation (in the US and Canada) and after orthopedic surgery (in Europe and Canada). Dabigatran has a predictable pharmacokinetic profile, allowing for a fixed-dose regimen without the need for routine coagulation monitoring.1 The half-life of dabigatran in healthy subjects is 12-17 hours. Maximum concentration (Cmax) occurs 1 hour post-administration in the fasted state, with a delay of ap-proximately 2 hours after a high-fat meal. In certain clinical situations, such as critical organ bleeding (eg, intracerebral), emergency surgery, or when switching treatment from warfarin to dabigatran or vice versa, clinicians need to determine the anticoagulant status of patients re-ceiving dabigatran. The best tests to quantitatively evaluate the anticoagulant effects of dabigatran are the thrombin and ecarin clotting times, which are not readily available in clinical practice.2,3 The activated partial thromboplastin time (aPTT) provides a readily available qualitative assessment of the anticoagu-lant status of patients receiving dabiga-tran.1 The maximum approved dabiga-tran dose, 150 mg twice a day, elevates the median peak aPTT to approximately twice that of control, with trough levels of 1.5 times con-trol (12 hours postdose).2 A normal aPTT level suggests that the anticoagulant effect is absent.1 The international normalized ratio (INR) and prothrom-bin time (PT), the other readily available coagulation as-says, do not accurately reflect the anticoagulant status of dabigatran-treated patients.1 At peak therapeutic concentra-tions of dabigatran (Cmax 184 ng/mL; [64-443]), the INR is only modestly elevated, in the range of 1.1 to 1.7.2 Supratherapeutic concentrations of dabigatran (400-600
