More than one in five women in Canada features this condition, which is associated with various mental, physical, and reproductive health challenges. Given the limited effectiveness of interventional approaches targeting this population—primarily focused on weight loss—this research aims to understand how a healthy future pregnancy is socially represented by overweight women and their partners. Grounded in the theoretical framework of social representations, this descriptive, interpretive and qualitative research explores in depth the shared socio-cognitive constructions that determine preconceptional health behaviors among women with excess weight and their partners. The methodological approach involved 16 women (mean body mass index of 36.4 kg/m²) and 7 male partners, recruited in Quebec. Data collected through individual semi-structured interviews and sociodemographic questionnaires underwent rigorous thematic analysis following Braun and Clarke's approach. The findings identify three shared conditions underlying the production and circulation of social representations on a future healthy pregnancy: (1) stigmatization at three levels (medical, social, and internalized) constituting a fundamental barrier to pregnancy preparation; (2) constrained preconceptional temporality with a dual urgency: the urgency to lose weight and reproductive urgency; and (3) specific needs of couples concerned with maternal excess weight. In-depth analysis revealed two distinct representational configurations among women. A predominantly negative social representation, associated with higher BMI and history of miscarriages, which is characterized by pronounced stigmatization and increased reproductive anxiety. Conversely, the positive representation revolves around resilience mechanisms and a holistic approach to health. Partners, occupying a peripheral position, manifest a more pragmatic vision, while the interactional dynamics within the couple demonstrate a process of co-construction where the feminine perspective predominates. These findings suggest the relevance of preconceptional interventions integrating psychosocial and relational dimensions, respecting women's autonomy while valuing active partner involvement. This research makes an original contribution through its woman-centered perspective on the preconceptional period, and its dyadic approach often neglected in previous studies. Its implications are manifold: orienting reflection toward a more inclusive preconceptional health prevention model, providing insights for improving clinical interventions aimed at optimizing the health of women of reproductive age and their partners, and contributing to reproductive justice by questioning the dominant biomedical approach.
