(227) Concept Development for a Novel Peyronie’s Disease Questionnaire for Men Who Have Sex With Men

Abstract Introduction Peyronie’s Disease (PD) exerts a substantial negative impact on the psychosocial well-being, sexual health, and overall quality of life in most patients. The Peyronie’s Disease Questionnaire (PDQ) was developed to capture the physical and psychosocial impacts of PD. The PDQ was validated in heterosexual men and is not applicable to men who have sex with men (MSM). Research suggests that gay men with PD may experience a higher degree of psychosocial distress compared to their heterosexual counterparts. Objective Our aims are (1) to convene a panel of PD experts to identify potential areas of concern unique to the MSM population with PD, (2) to understand the impact of PD on sexual health, quality of life, and relationship satisfaction in the MSM population using qualitative interviews, and (3) to develop a novel questionnaire for validation focusing on patient concerns and PD attributes in the MSM population. Methods A panel consisting of 12 national experts in PD was convened to discuss unique issues affecting the MSM population and to develop important domains for use in this questionnaire. These domains were then utilized to generate draft questions to be tested in the qualitative interviewing phase. The draft questionnaire was circulated amongst the experts, and after multiple iterations a finalized version was prepared along with an interview guide to be utilized by the interviewer to generate discussion of the questions with patients. Results Domains of importance to the MSM population developed by the expert panel included: anal intercourse, outercourse, pain, appearance, sexual function, sexual satisfaction, and distress/bother. Unique areas of interest in this population include whether patients engage in receptive and/or penetrative anal intercourse, whether PD has led to a change in sexual role, and the mirror concept (comparison of their penis to that of a partner). In total 45 Likert-scale questions were generated by the panel for use in the qualitative interview phase. Conclusions MSM patients with PD experience substantial impacts on physical, psychosocial, and sexual health, which is not able to be assessed using the existing PDQ. It is therefore important to generate a questionnaire for this population and to be able to track symptoms pre- and post-treatment. Thus far, we have generated a draft questionnaire focusing on sexual function, appearance, pain, and distress. Next steps including conducting qualitative interviews with a diverse set of 12-15 patients with PD who identify as MSM, which will subsequently undergo psychometric analysis and be utilized to edit the draft questionnaire before formal validation.
