EP232 A Systematic Review of Clinical Outcomes After Periacetabular Osteotomy in Patients with Borderline Hip Dysplasia

Abstract Background The impact of periacetabular osteotomy (PAO) on clinical outcomes in patients with borderline hip dysplasia (BHD) remains uncertain. The purpose of this study was to perform a systematic review of clinical outcomes in patients with BHD undergoing PAO. Methods A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines by searching PubMed, the Cochrane Library, and Embase to identify English language studies reporting on clinical outcomes in patients with BHD undergoing PAO. The search phrase used was: periacetabular osteotomy borderline hip dysplasia. The outcomes assessed were conversion to total hip arthroplasty (THA), return-to-sports (RTS) rates, and patient-reported outcomes (PROs) including the modified Hip Harris Score (mHHS), Hip Outcome Score (HOS), Subjective Hip Value (SHV), Hip Disability and Osteoarthritis Outcome Score (HOOS), Short-Form Health Survey (SF-12 or SF-36), International Hip Outcome Tool (iHOT-12 or iHOT-33), and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores. Results Eleven studies (2 Level III, 9 Level IV) out of 198 met the inclusion criteria, including a total of 657 hips with an average patient age of 26.9 years and a mean follow-up time of 48.1 months. The average body mass index was 23.3 kg/m2 and the overall percentage of males was 13.0%. Four studies demonstrated a significant improvement in the mHHS, with a total weighted mean increasing from 62.6 preoperatively to 82.9 postoperatively. Three studies found significant improvement only in the physical domain of the SF-12 or SF-36, with a total weighted average increasing from 37.8 preoperatively to 47.6 postoperatively. Two studies found significant improvement in iHOT scores. Two studies found significant improvement in the SHV score. The overall reoperation rate was 9.5%, with a rate of conversion to THA of 0.2%. The majority of reoperations occurred due to irritation from implants. One study found the RTS rate among preoperatively active patients to be 92.5%, with most resuming sports activity within six months. Conclusion In patients with BHD undergoing PAO, there is consistent improvement in most PROs along with a low rate of reoperation and conversion to THA.
