Challenges related to job acquisition among individuals with schizophrenia

Introduction: Work is considered one of the main forms of social organization; however, only few individuals with schizophrenia have work opportunities. This study aimed to identify and analyze the relationship between symptomatology and work acquisition and permanence, as well as the perceptions about positive and negative experiences related to work experiences. Methods: Two studies were conducted: the first one, with quantitative approach and longitudinal follow-up of eighteen months. Fifty-three individuals with schizophrenia in outpatient treatment were included. All of them participated in a prevocational training group. At the end of the group and eighteen months later, socio-demographic, clinical data and the occupational history were collected. Furthermore, instruments to evaluate symptoms (PANSS), depression (Calgary), severity of the disorder (CGI), functionality (GAF), personal and social performance (PSP) and cognitive functions (MATRICS battery) were used. The second study used a cross sectional and with a qualitative approach. Were included 11 subjects among participants of Study I. All of them answered one question about the positive and negative aspects of work experiences during the follow up. From the answers 4 representative categories were defined. The Thematic Analysis Technique was used for discussing the results. Results: The first study indicated that those who had some work experience (n=19) presented lower scores on PANSS, Calgary, GAF, CGI and PSP scales (p<0.05) compared to those who did not work. Among those who worked, there was a slight positive symptom worsening (positive PANSS). The second study reflected about the importance of care considering the four thematics cathegories: overload and lack of information/guidance at work, payment, responsibility, hope and overcoming and health care. Conclusions: To promote professional inclusion, some actions are important, such as to encourage new ways of understanding schizophrenia and dealing with the stigma and lack of information about the disorder. Furthermore, work engagement requires an understanding of the impact of the disorder on job inclusion and maintenance. Lastly, support for work acquisition entails avoiding occupational risks and minimizing the interference of employment in symptomatic worsening. While considering such conditions, it is possible to argue for the construction of treatment strategies that enable social participation, ordinary social status and advocacy for these individuals.
