A case study of professional identity formation of hospital-trained dietitians in Ontario

Female gendered professions such as nurses, occupational therapists, physiotherapists and social workers strive to protect and expand their professional image. Like all female dominated professions, the dietetic profession in Canada has struggled to protect and expand their professional image. The healthcare budget cuts, the accessibility and provision of nutrition information (whether accurate or not) on the web, and the encroachment of other professions providing nutrition care are some of the challenges facing dietitians. This study explores how dietitians develop their professional identity during their practicum placements, including the factors that influence their professional identity formation at the start of and during their professional education. Literature shows that profession-specific education has a positive impact on the development of professional identity. University education establishes the theoretical underpinnings and the knowledge and skills needed to practice in the profession. Practicum placements are a required component of dietetic education and training. The experiences during these placements allow students to translate theory into practice and to learn the importance of professional socialization. Students’ practicum experiences may strengthen or distance and their perception of and fit within the profession. Three dietitians who graduated between 2016-2019 from a post-degree internship and combined masters practicum program were interviewed using a semi-structured protocol. The transcribed data were coded for common categories. The results from this qualitative case study show the evolution of professional identity formation. Three factors of professional identity formation were explored: internal; external; and curriculum education (Ellis & Hogard, 2020). The study participants had contrasting experiences that impacted their professional identity formation as it evolved along their path to becoming qualified as dietitians. Tension felt with their practicum preceptors had a strong impact on their professional identity because it was incongruent with the participants’ ideals of being a dietitian. Clarity of professional identity was evident following completion of their training when the newly qualified dietitians were employed in hospitals. Further exploration of dietitians who completed a practicum placement in various areas of dietetics practice could extend the examination of how their professional identity (PI) formation is formed.
