Chronic Obstructive Pulmonary Disease (COPD) is a progressive condition and a leading cause of mortality and disability globally. It ranks as the fifth leading cause of death in Canada, with a population-adjusted prevalence rate of 9.5% among Canadians over 35. COPD predominantly affects older adults who often present with concurrent conditions, collectively termed multimorbidity. The coexistence of multimorbid conditions is strongly associated with increased healthcare utilization and poorer health outcomes in patients with COPD. This thesis investigates patterns in the development of multimorbid conditions in individuals with COPD. Aiming to improve healthcare management for controlling multimorbidity progression in patients with COPD, this thesis also examines the impact of health service utilization patterns on COPD multimorbidity-related outcomes. The first study found that a majority of patients with COPD (72.5%) residing in Ontario had one or two pulmonary or non-pulmonary multimorbid conditions at the time of study inclusion. The most prominent multimorbid conditions coexisting with COPD were anemia, asthma, CVD, diabetes, gastrointestinal disorders, and sleep apnea. However, associations between these multimorbid conditions were weak indicating a diffuse pattern of multimorbidity. The second study revealed that high continuity of non-acute care offered moderate protection to patients with COPD against the development of new multimorbid conditions (hazard ratio [HR]: 0.81 [0.75, 0.87]). A higher proportion of non-acute healthcare services was associated with a significantly lower risk of developing new multimorbidity (HR: 0.31 [0.18, 0.44]). The third study, focusing on the COVID-19 period, found that SARS-CoV-2 raised the multimorbidity burden in patients with COPD by approximately 20%. It also showed that the rate of not attaching to a primary care physician (PCPs) increased by 2.9% amongst patients with COPD at the onset of the pandemic and that patients with COPD without PCP had lower rates of multimorbidity diagnoses both before and after the pandemic. Resources should be increasingly allocated to non-attached patients with COPD to improve early disease detection and support chronic disease management. Strategies for managing COPD should be tailored to the patient’s clinical health status and disease severity, ideally involving collaboration with an integrated care team improve care continuity.
