Understanding Human Resource Development in a Fragile and ‘Conflict-Affected’ Setting: A Feminist Analysis of Human Resources for Health Policy Initiatives for Midwives, Nurses, and Physicians in Afghanistan (2002–2020)

Historically, Afghanistan has struggled with a shortage of female healthcare providers and limited access to women's healthcare, a situation exacerbated by the Taliban Regime (1996-2001). The post-conflict period brought the opportunity to address these issues in alignment with the United Nations' Millennium Development Goals. This retrospective policy case study analyzed policy documents of national and international partners to examine the construction of gender equity in Human Resources for Health (HRH) policies for midwives, nurses, and physicians. Kingdon's Multiple Streams Framework, within Mcphail's feminist policy framework, guided the analysis of policy actors' roles and construction of gender equity in policy initiatives.National political and global health imperatives shaped the policy reforms of post-conflict reconstruction. International development partners contracted health service delivery to NGOs on behalf of the Ministry of Public Health. These services drove subsequent HRH development over what I have termed the three progressive eras of policy development in the reconstruction period: humanitarian assistance (2002-2007), institutional development (2007-2014), and civil society development (2007-2014). Midwifery emerged as the success story of the policy reforms, acknowledged for significantly reducing maternal mortality, increasing women in the workforce, and legitimizing Afghanistan as a regional leader in midwifery. However, improvements in women's health indicators beyond reproductive health were limited, with poor outcomes remaining for tuberculosis, mental health, and non-communicable diseases. The success of midwifery can be attributed to the early institutionalization of safe motherhood initiatives into the healthcare system. Conversely, national and international stakeholders, including professional associations, failed to seize the opportunity during the reconstruction to promote women in nursing and medicine and expand women's access to comprehensive care. With the singular policy focus on women's reproductive health, initiatives served to reinforce patriarchal structures that designated women as mere reproductive agents and failed to address broad health issues for women. Moreover, poor coordination among various stakeholders during policy formulation and cultural limitations on hetero-social interaction during implementation likely contributed to maintaining the paucity of women nurses and physicians. The study highlights the need for gender-sensitive and inclusivity-based, comprehensive planning and implementation of policy initiatives to enhance women's health and professional profile in Afghanistan's health system.
