Aerobic exercise during stroke rehabilitation: factors affecting use of aerobic exercise by physiotherapists and participation of people with stroke during rehabilitation

Although aerobic exercise is a critical component of stroke rehabilitation, rate of participation in aerobic exercise during rehabilitation is low. While many studies have explored the barriers and facilitators to the use of aerobic exercise from the physiotherapists’ perspective, only study has explored this from patients’ perspectives. Additionally, no study has investigated the effect of patient related individual-level factors on aerobic exercise participation. The objective of this work was to understand factors affecting the use of aerobic exercise and participation in aerobic exercise during stroke rehabilitation. The first study, a qualitative descriptive one, found that physiotherapists’ behaviours regarding use of aerobic are not binary and are better understood on a continuum. This study identified 3 themes for the factors that affect physiotherapists’ use of aerobic exercise during stroke rehabilitation. These themes included:1) physiotherapists’ perspectives and practices around aerobic exercise; 2) profiles of people with stroke, as well as their goals and their exercise modality preferences; and 3) influence of health system priorities, rehabilitation intensity policy, and resources. The second study, another qualitative descriptive one, found that people with stroke participated in any exercise that their physiotherapist prescribed to them (including aerobic exercise) even if it was difficult for them. This study identified six themes 1) having an exercise program routine and trusting the physiotherapist during rehabilitation facilitated doing exercise ; 2) emotions can make exercise during rehabilitation more or less difficult; 3) limited physical ability post stroke leads to poor exercise self-efficacy and sense of control; 4) knowledge of exercise affects perseverance in doing exercises during rehabilitation; 5) personal identity affects perseverance in doing exercises during rehabilitation, and 6) Environmental factors facilitate exercise performance (consisting of two subthemes of supportive social environment promotes exercise participation ; and more resources (e.g., time, space, staff, other programs) facilitate exercising during and after rehabilitation). The third study, which was a prospective cohort study, found that the people with stroke who participated in aerobic exercise during rehabilitation and those who did not, are not equivalent to each other with regards to their exercise self-efficacy, perceived barriers to exercise, past exercise history and depressive symptoms. Our findings suggest that interventions to increase the uptake of aerobic exercise during stroke rehabilitation could target healthcare professionals and system-level barriers, rather than patients. Also, individualizing exercise programs based on exercise self-efficacy, perceived barriers to exercise, past exercise history and depressive symptoms may make it less challenging for people with stroke to exercise during rehabilitation.
