В хронологічній послідовності відтворено класифікаційну номенклатуру кардіоміопатій на підставі морфофункціональних характеристик та клінічної маніфестації. Подано сучасне визначення дилатаційної кардіоміопатії відповідно до рекомендацій міжнародних експертів. В огляді увага приділена методології верифікації фенотипів дилатаційної кардіоміопатії за допомогою електрокардіографії, ехокардіографії, магнітно-резонансної томографії, ендоміокардіальної біопсії. Поряд з мультимодальною візуалізацією міокарда для диференційної діагностики ізольованої кардіоміопатії з вторинними ураженнями серця, системними синдромними фенокопіями, наведена клінічна значущість застосування додаткових методів обстеження хворих із стандартною оцінкою екстракардіальних патологічних проявів, результатів лабораторних досліджень, коронарної ангіографії, а також необхідність генетичного тестування та сімейного скринінгу. Archived: https://doi.org/10.5281/zenodo.17523144 Abstract This article provides a comprehensive overview of Dilated CardioMyopathy (DCM) based on an analysis of scientific literature from MEDLINE/PubMed, Web of Science, Scopus, and Google Scholar. It traces the historical evolution of myocardial pathology understanding, from early post-mortem descriptions to the establishment of "cardiomyopathy" as a distinct clinical entity and the subsequent development of morpho-functional classifications. The review presents the modern definition of DCM per international expert recommendations, characterized by left ventricular dilation and systolic dysfunction not explained by abnormal loading conditions or coronary artery disease. A core focus is the methodology for verifying DCM phenotypes, detailing the roles of electrocardiography, echocardiography, cardiac magnetic resonance imaging, and endomyocardial biopsy. The article further expands on the use of multimodal imaging for differential diagnosis against secondary heart damage and systemic phenocopies. It also covers the clinical relevance of supplementary assessments, including laboratory tests, coronary angiography, genetic testing, and family screening. The instrumental diagnostic workup is analyzed in depth, emphasizing the prognostic value of specific electrocardiography patterns, echocardiography for assessing structure and function, and the unique capability of cardiac magnetic resonance imaging with late gadolinium enhancement to detect myocardial fibrosis – a key predictor of adverse outcomes. The indications for endomyocardial biopsy in confirming specific etiologies are also outlined. Finally, the paper covers differential diagnosis principles and concludes with a perspective on integrating genetic testing into clinical practice for risk stratification and family management.
