The main goal of this thesis was to better understand the first service recurrence in youth protection experienced by First Nations and non-Indigenous children (0-17 years old) served by the CISSS de la Côte-Nord. This research is the result of a collaboration with partners in the region and benefited from the contribution of an advisory committee throughout the process, made up of representatives from the CISSS and social services of Innu communities in the region. Administrative data (2002-2014) from the youth protection services of the CISSS de la Côte-Nord were used to respond to two sub-objectives. A total of 4,246 children experienced a first youth protection investigation in Côte-Nord during this period. The first research objective was to identify ethnoracial groups whose trajectories would be analyzed more specifically in the second objective. The second objective was to identify the associations between certain characteristics noted in the files and the decisions taken at three major decision points in youth protection: first episode of service (logistic regression models), closure of the first service episode (Cox proportional risk models) and first service recurrence (Cox proportional risk models). These analyzes were conducted for all of the children who underwent a first youth protection investigation (N = 4246), as well as for First Nations children residing in a First Nations community (n = 1340) and children from the majority non-Indigenous group in the region (n = 2142). These analyzes made it possible to note the factors associated with a first episode of services and with a first recurrence of youth protection services for First Nations children residing in First Nations communities, allowing the identification of reflections more specific to this group. Analyzes have shown that the initial youth protection trajectories of First Nations children living in a First Nations community began earlier and lasted longer than those of children from the majority group. This study also highlighted the preponderance of negligence (including serious risk) among the reasons for intervention (78% of open cases had negligence for primary or secondary reason) and the chronicity of contact with youth protection, particularly at the family level (the children's family had already been assessed for another child in 48% of new cases opened; this was the case in 60% of recurrent cases). Taken together, these results indicate situations of needs that persist over time and to which current services, as much in youth protection as the other services available, do not seem to be able to respond in their current structure and with their current resources. These results could feed into discussions and reflections related to the governance and self-determination of First Nations in matters of youth protection, very current issues at the Canadian and Quebec level, especially in the context of the coming into force of the Act concerning First Nations, Inuit and Métis children, youth and families and its legal challenge by the Government of Quebec, as well as in the context of the calls to action and the steps taken following the Viens Commission report on relations between Aboriginals and certain public services in Quebec
