Abstract
Las neoplasias malignas de la via biliar distal (colangiocarcinoma) aun son algo poco habitual con un pronostico desfavorable. La inmensa mayoria de estos canceres son adenocarcinomas que invaden preferentemente las estructuras adyacentes y drenan en los ganglios linfaticos locales. Dado el pequeno diametro del coledoco, no resulta posible el reconocimiento tempranodel tumor con las actuales tecnicas de imagen. Por eso, estos pacientes suelen acudir con sintomas, y la mayoria de ellos, con un estado avanzado de la enfermedad. Estos canceres crecen en general demanera perpendicular y horizontal a lo largo del coledoco y, por eso, los tumores que se detectan con las pruebas de imagen tienden a infravalorarse y aparecen, a menudo, mas extendidos en la exploracion quirurgica. La relacion anatomica entre la porcion distal de la via biliar y el pancreas, el duodeno, la vena porta y la arteria hepatica tambien puede incrementar las dificultades tecnicas de la extirpacion tumoral.
