Abstract
Bevezetés: A velőcső-záródási rendellenességek a leggyakoribb súlyos veleszületett fejlődési rendellenességek közé tartoznak. Többségük multifaktoriális eredetű, mind \nkörnyezeti, mind genetikai faktorok befolyásolják megjelenését. A folsavbevitel meghatározó környezeti tényező, mely csökkenti e rendellenesség incidenciáját. 2003-ban kanadai kutatók megfigyelték, hogy amióta országukban folsavval dúsított liszt került forgalomba, a neuroblastoma előfordulása 60%-kal csökkent. \nCélkitűzés: A szerzők célja a terhesek folsavszedési szokásainak felmérése és a hazai ajánlásokban foglaltakkal való összevetése, valamint a folsavszedés, \na velőcső-záródási rendellenesség és a neurobólastoma \nelőfordulási gyakorisága közötti összefüggés vizsgálata volt. \nMódszertan: A folsavszedési szokásokat kérdőívekkel vizsgálták, melyeket a védőnők feljegyzéseik alapján töltöttek ki. Az adatokat számítógépben rögzítették, majd statisztikai módszerekkel dolgozták fel. A velőcső- \nzáródási rendellenesség gyakoriságát a Veleszületett \nRendellenességek Országos Nyilvántartó adatai alapján \nhatározták meg. Összehasonlításul az Országos Egészségbiztosítási Pénztár szekszárdi adatbázisát használták. A neuroblastoma incidenciájának meghatározása a Magyar Gyermektumor Regiszter adatain alapult. Hazánkban a védőnők az 1980-as évek óta ajánlják a folsav szedését. A \nSzülészeti és Nőgyógyászati Szakmai Kollégium a hivatalos \najánlást 1997-ben jelentette meg. Jelen munka a 2001- \nben, a 2002-ben és a 2003-ban várandósok folsavszedési \nszokásait mérte fel. A három esztendő alatt Magyarországon \n285 884 gyermek született. A szerzők 271748 várandós \nadatait gyűjtötték be, ami 95%-os országos szintű felmérést \njelent. Az adott évek vizsgálatát az magyarázza, hogy Magyarországon a neuroblastoma 45%-a 1 év alatti, szintén 45%-a 1–5 év közötti gyermekekben fordul elő (Magyar Gyermektumor Regiszter). \nEredmények: Az adatok statisztikai elemzése után megállapítható, hogy Magyarországon a várandósok 69%-a rendszeresen szedett valamilyen folsavat tartalmazó készítményt. A folsavat szedő terhesek 93%-a a folsavszedést a 7. terhességi hét után, az első védőnőhöz fordulást követően kezdte meg, bár a velőcső záródása a terhesség 28. napján befejeződik. A folsav napi dózisa a rendszeresen szedők 85%-ánál napi 5 mg alatt volt. A rendszeres folsavszedés ellenére a velőcső-záródási rendellenességek gyakorisága változatlan volt. A neuroblastoma előfordulása évenként változó, enyhén növekvő tendenciát mutatott. Következtetések: Megállapítható, hogy a folsav szedése hazánkban későn, a periconcepcionalis időszak után kezdődik, és a terhesek 85%-a kevesebbet szed, mint az egyes irodalmi adatok szerint ajánlott napi 5 mg. A tervezett terhességek számának növekedése lehetővé tenné, hogy a kismamák hamarabb \nkezdjék meg a folsav szedését, azonban az optimális hatás a külföldi példáknak megfelelően csak a folsav liszthez történő keverése esetén lenne várható. \n \nIntroduction: Neural tube defects are common major congenital anomalies. Folic acid supplementation has been shown to reduce the incidence of neural tube defects. In 2003, incidence of neuroblastoma has decreased with 60% in Canada as a result of enriched cereal grain flours with synthetic folic acid. The aim of this study was to investigate the effect of the practice of the folic acid intake by pregnant women (based on the Hungarian recommendation) to the incidence of neural tube defects and neuroblastoma. \nMethods: The practice of folic acid supplementation was examined by questionnaires filled according to the documentation of health visitors. The data were worked up by computer. The incidence of neural tube defect was obtained from the data of the Hungarian Congenital Anomalies Registry, however, the data of National Health Insurence Company are also given. The incidence of neuroblastoma was the data of the Hungarian Pediatric Tumor Registry. Regular \nfolic acid intake has been recommended to pregnant women \nin Hungary, since the eighties of the last century. An \nofficial protocol had been released by the Obstetric and \nGynecologic Professional Board in 1997. In this paper, the \nauthors report the Hungarian pregnant women’s folic acid intake in years of 2001, 2002 and 2003. These years were choosed, because according to the data of the Hungarian \nPediatric Cancer Registry 45% of the neuroblastoma cases are less than 1 year old, and 45% of them are 1–5 years old at the time of diagnosis. The authors succeded to collect the data from 95% of the pregnant women during these years (271 748 women). \nResults: Based on the statistical analysis of the collected data, 69% of the pregnant women were regularly taking folic acid products in Hungary, however, the dose of the daily intake was below 5 mg. 93% of the pregnant women started \nthe folic acid intake after their 7th weeks of pregnancy. \nThe incidence of neural tube defects was constant, and the incidence of neuroblastoma slightly increased during the above mentioned period. \nConclusions: This work highlighted that, the folic acid intake to prevent neural tube defects was started too late, because the formation of neural tube is finished on 28. day of pregnancy. 85% of the pregnant women used less amount of folic acid than 5 mg/day. The increasing number of planned pregnancies would allow to start folic acid intake earlier. However, based on international experience, the enrichement \nof cereal grain flours with synthetic folic acid could \nprovide optimal results.
