Abstract
Accessory pathways that conduct in the antegrade direction produce ventricular preexcitation (delta wave). Accessory pathways are capable of sustaining for both supraventricular tachycardia as well as potentially malignant arrhythmia. An accessory pathway mediated tachycardia must be differentiated from atrial tachycardia and atrioventricular (AV) node reentrant tachycardia using pacing maneuvers during tachycardia. Once the diagnosis has been established, accurate localization and mapping of the accessory pathway requires a detailed understanding of electrocardiogram (ECG) delta wave vector analysis combined with meticulous mapping of the AV valve annuli. Excellent short- and long-term success rates coupled with low complication rates make ablation an ideal treatment modality.
