Abstract
L'histoire des trente dernières années de la psychiatrie nord-américaine pointe les difficultés posées par le traitement du psychotique en institution hospitalière mais surtout celles rencontrées par le maintien de son articulation sociale. C'est dans un contexte de recherche de solutions et à partir des besoins repérés au Centre hospitalier Robert-Giffard à Québec qu'un groupe de professionnels du GIFRIC a proposé d'ouvrir, au coeur de la ville, un Centre psychanalytique de soins globaux pour les psychotiques. En opération depuis 1982, le «388» a pour objectif la réarticulation sociale des psychotiques à une vie sociale satisfaisante pour eux, selon leurs moyens de coexistence avec autrui. C'est une structure extra-hospitalière permettant à la fois de traiter la crise et de procurer des soins à long terme. Une problématique psychanalytique de la psychose y détermine la dispension des services et des soins. Nous tracerons ici le portrait de la clientèle, des services offerts, du traitement, de l'évolution des patients et des problèmes posés par l'évaluation d'une telle structure nouvelle.
