Transfusion-associated graft-versus-host disease (TA-GVHD) is an acute illness characterised by fever, rash, pancytopenia, diarrhoea and liver dysfunction which begins 4-30 days (median 8-10 days) after transfusion and results in death within three weeks from symptom onset in over 90% of the cases. Patients at increased risk for TA-GVHD include those who are immune impaired and those receiving blood components donated from blood relatives. Fetuses and neonates have immature immune systems and may be at increased risk of TA-GVHD. In neonates, most cases of TA-GVHD reported are in those with congenital immunodeficiency or who received products from related donors. Transfusion-associated microchimerism (MC) has been reported mostly in trauma patients, but has also been reported in sickle cell disease and thalassaemia patients. Irradiation of products prevents TA-MC. Leucocyte reduction of blood products alone does not decrease the incidence of TA-MC among trauma patients. In addition, TA-MC can be sustained for decades after transfusion.
