La citoyenneté est un objet fragile, source d’interrogations pour les praticiens, les chercheurs, mais aussi pour les usagers. Cet article est le résultat d’un dialogue tenu lors du Deuxième colloque international francophone sur la dépendance aux opioïdes (Paris, les 4 et 5 novembre 2010), qui a réuni une chercheure québécoise et un représentant français de l’autosupport des usagers de drogues. Tous deux ont tenté de répondre à la question posée : La citoyenneté de l’usager de drogues peut-elle constituer un facteur déterminant de la réussite des traitements de substitution aux opiacés (TSO) ? La réponse est ambivalente. Elle témoigne à la fois de la virtualité positive de la dimension citoyenne dans l’espace thérapeutique, mais aussi des limites de cet exercice dans le cadre spécifique des TSO. Depuis 20 ans, les pouvoirs publics, en France comme au Québec, renforcent les dispositifs légaux qui favorisent la participation des personnes sans réussir à impliquer à un niveau significatif des représentants du secteur des addictions. Cette situation est le résultat d’un déficit de compétences qui mériterait d’être pris en compte, mais aussi d’une stigmatisation particulière des usagers de TSO, toujours suspectés de se complaire dans la toxicomanie.
