Abstract
Heparin is a sulfated glycosaminoglycan with a range of molecular weights from 3000 to 30,000 Daltons. It is most often prepared with extractions from porcine mucosa. Unfractionated heparin (UFH) was first used in the early 1940s and was the only immediate-acting anticoagulant available until the 1980s when low molecular weight heparin (LMWH) was introduced. LMWHs are prepared from UFH by chemical or enzymatic depolymerization and are about one-third the molecular weight component of UFH. The smaller the molecular weight chains, the higher the antifactor Xa activity and lower the anti-IIa activity. This results in less interference on the activity assays of coagulation. Both heparins can be measured using Xa inhibition assays but only UFH can be monitored with activated partial thromboplastin time (aPTT) or thrombin (IIa) inhibition assays. Blood collection and preparation is critical to assuring accurate heparin assays. Current assays have been criticized for not adequately measuring the biological effect of the drugs. Newer assays may overcome such criticism.
