Les travaux qui conçoivent les prisons comme des instruments de maintien de l’ordre social soulignent leur permanence et leur résistance au changement. Cet article poursuit cette réflexion en revenant sur le processus d’appropriation et de régulation des tests de dépistage du VIH comme instrument de maintien de l’ordre interne des prisons. Antérieurs à l’épidémie de sida, les premiers usages des tests VIH s’inscrivent dans une logique de police sanitaire. Cependant, l’inadéquation entre ce dispositif et les exigences spécifiques de la lutte contre le sida conduit les pouvoirs publics à une régulation spécifique de ces instruments, dans le cadre notamment de la réforme de la médecine pénitentiaire. Cette dernière entérine le rejet d’un recours systématique et coercitif aux tests VIH pour les cantonner dans un registre axé sur le diagnostic individuel et le soin. Mais cet usage clinique masque un usage plus sécuritaire qui se manifeste dans les pratiques quotidiennes des acteurs du système carcéral. Ainsi, les dimensions sécuritaires et cliniques tendent à s’imbriquer, voire à se confondre.
