Resume Objectif : Discuter des nouveautes relatives au traitement de l’insuffisance cardiaque (IC) depuis 2001. Source des donnees et selection des etudes : Une recherche dans Medline en francais et en anglais entre janvier 2001 et mars 2004 ainsi que des abreges des congres en cardiologie a ete effectuee et les articles les plus pertinents ont ete selectionnes. Analyse des donnees : Depuis 2001, un nombre important d’etudes concernant le traitement et le diagnostic de l’IC ont ete publiees. Celles-ci ont elargi la place dans la therapie des patients avec IC des betabloqueurs et des antagonistes des recepteurs de l’angiotensine II. De plus, un nombre important d’etudes supportent une utilisation plus repandue des defibrillateurs implantables et l’arrivee dans l’arene clinique de la therapie de resynchronisation cardiaque. Finalement, sur le plan diagnostique, le B-type, ou brain natriuretic peptide, represente un ajout important permettant d’ameliorer l’evaluation des patients se presentant a l’urgence avec comme plainte principale de la dyspnee chez qui un diagnostic d’IC est soupconne. Conclusion : Le traitement des patients atteints d’IC a grandement evolue au cours des deux dernieres decennies et s’est beaucoup complexifie. Ainsi, il apparait fondamental que le pharmacien occupe un role de premier plan dans le traitement de ces patients. Abstract Objective: To discuss new developments in congestive heart failure that have occurred since 2001. Data sources and study selection: Search of MEDLINE (June 2001 - March 2004) was performed as well as a review of abstracts from cardiology meetings. All articles identified from the date sources were evaluated and all information deemed relevant was included in this review. Data synthesis: Since 2001, a number of studies regarding the diagnosis and treatment of congestive heart failure have been published. These studies have broadened the role of beta-blockers and angiotensin II-converting enzyme inhibitors in the treatment of congestive heart failure. Furthermore, a number of studies acknowledge the use of implantable defibrillators, as well as the use of cardiac resynchronization. Finally, on the diagnostic level, B-type, natriuretic peptide represents and important addition. It allows better evaluation of patients admitted to the emergency room complaining of dyspnea, and in whom congestive heart failure is suspected. Conclusions: Treatment of congestive heart failure has greatly evolved during the last two decades, and it has also become more complex. Therefore, it is important that pharmacists play a role in the treatment of these patients.
