Abstract
La taquicardia sinusal inapropiada (TSI) es una arritmia rara que se caracteriza por un incremento exagerado en la frecuencia cardiaca asociado con demandas fisiologicas comunes. Generalmente, la TSI se inicia por cambios posicionales, ejercicio minimo y estres psicologico. La etiologia de la TSI no esta completamente esclarecida. Sin embargo, algunos de los mecanismos propuestos incluyen: aumento del automatismo del nodulo sinusal, alteraciones en la funcion autonomica mediadas por un incremento en la actividad simpatica o una disminucion de la actividad parasimpatica, y anormalidades en el control barorreflejo. Recientemente se ha observado un aumento de autoanticuerpos betaadrenergicos en pacientes con TSI. La presentacion clinica de la TSI es variada: desde episodios cortos de palpitaciones asociadas con disnea, dolor precordial atipico, cefalea, fatiga y ocasionalmente sincope y presincope hasta la taquicardia incesante incapacitante. La TSI se diagnostica por exclusion. Es necesario descartar otras causas de taquicardia sinusal antes de establecer el diagnostico. La evaluacion de la funcion autonomica es indispensable para el diagnostico de la TSI. El tratamiento de la TSI esta basado en los hallazgos autonomicos y una atencion multidisciplinaria en la que se incluye la rehabilitacion cardiaca. El control y la restitucion de la funcion autonomica son indispensables. El pronostico es benigno y se requiere un seguimiento periodico para ajustar la terapia.
