Hydromyelia is a collection of fluid within the spinal cord connected to the fourth ventricle almost always associated with hydrocephalus or certain other types of brain-related birth defects. Its diagnosis by sonography is well documented in neonatal period, but not prenatally. We report a case of cervical hydromelia associated with Chiari malformation II in a dichorionic diamniotic twin pregnancy. Findings at 21 and 23 weeks ultrasound and MRI exams are discussed (image). To the best of our knowledge, this is the second case to be described in the literature of prenatal diagnosis of hydromyelia.
