Abstract
External-beam radiotherapy (RT) in the management of papillary thyroid cancer can be subdivided regarding its purpose into adjuvant, radical, and palliative. The intent of adjuvant therapy is to improve the results of standard therapy that consists of surgery and radioactive iodine; however, its exact role is controversial. Unlike radioactive iodine, external-beam RT is a local therapy, which confines its impact to cases where a local treatment is required to maximize local regional control of the disease beyond what can be achieved with optimal surgery and radioactive iodine. Therefore, RT is generally limited to patients considered to have a high risk of locoregional recurrence, e.g., older patients with significant extrathyroidal extension of disease, residual disease following surgery, or extensive lymphatic spread.
