Cemented vs. Uncemented Fixation in Total Knee Arthroplasty

Abstract
Total knee replacements (TKR) are most commonly inserted with the use of cement. Interest in cementless fixation persists in hope of improved outcomes. This review of the evidence finds that cemented TKRs demonstrate improved survivalship compared to cementless TKA; however, mid term functional outcomes appear equivalent if loosening doesn't occur. Differences in cost, complications and ease of revision are not well documented. The use of hydroxyapatite coating appears to improve bone ingrowth with cementless components. Interest in cementless fixation will likely remain high as efforts continue to improve long term (15+ year) survivalship of implants. Further long term data is required.
