Abstract
The majority of children with chronic medical problems do not have a psychiatric illness. However, the risk for psychological and social adjustment problems in those children is approximately twice the risk of healthy children. Additionally, comorbid psychiatric and pediatric medical problems contribute to increased health-care costs, less satisfactory outcomes, and increased diagnostic uncertainty. The consultation-liaison child psychiatrist's role is to help educate pediatric colleagues about the comorbidity of medical and psychiatric disorders and the importance of psychiatric consultation, and to work as part of the multidisciplinary team in order to provide comprehensive care for these children. This chapter examines (i) the epidemiology and characteristics of psychiatric disorders in medically ill children; (ii) reasons for psychiatric consultation; (iii) psychiatric assessment; and (iv) psychiatric sequelae of chronic medical problems. The role of integrated mental health care in a medical home is described, as part of the evolving model of coordinated care and health maintenance in settings of moderate or low acuity.
