While the clinical symptoms of diabetes mellitus vary and are not of themselves reliable indicators of the disorder, when they are present the diagnosis of diabetes can be confirmed by a single unequivocally elevated blood glucose measurement. Hyperglycemia by itself may be transitory and for the asymptomatic person at least one additional blood glucose measurement with a value in the diabetic range is essential for diagnosis. The oral glucose tolerance test is one of the most widely used and misused methods of diagnosis but may be difficult to perform and for epidemiological studies a single 2 h value after an overnight fast often suffices. The new diagnostic criteria for fasting plasma blood glucose concentrations and the revised classifications for diabetes proposed by the American Diabetes Association and the World Health Organization are discussed. The new etiological types that may result in diabetes mellitus include Type 1, Type 2, immune-mediated, genetic, pancreatic disorders, virally associated and gestational.
