Latar belakang: Hipertensi dapat mengganggu sirkulasi aliran darah otak yang dapat menyebabkan gangguan fungsi. Salah satunya dapat timbul gangguan fungsi kognitif. Tujuan: Menilai fungsi kognitif pada lanjut usia yang menderita hipertensi Metode: Jenis penelitian adalah observasional dengan desain belah lintang. Dilaksanan di poli rawat jalan instalasi geriatri Rumah Sakit Dr.Kariadi Semarang. Pada bulan april sampai mei 2014. Subjek penelitian adalah pasien rawat jalan di instalasi geriatri (n=49). Pengukuran tekanan darah dilakukan dengan tensimeter air raksa merek NOVA, sedangkan Fungsi kognitif diukur dengan tes Montreal Cognitive Assesment versi Indonesia (MoCa-Ina). Uji hipotesis menggunakan uji korelasi spearman. Hasil: Hasil pemeriksaan fungsi kognitif pada 49 subjek penelitian didapatkan 38 subjek mengalami gangguan fungsi kognitif, pada subjek yang saat ini menderita hipertensi atau yang kurang dari 5 tahun didapatkan hasil yang tidak signifikan (p=0.847). Sedangkan nilai p bermakna didapatkan pada subjek yang memiliki riwayat hipertensi lebih dari 5 tahun memiliki fungsi kognitif yang terganggu sebanyak 38 subjek, dengan persentase sebesar 77,5% dengan hasil yang signifikan (p=0.001) dengan rasio prevalensi sebesar 21,1. Kemudian fungsi kognitif yang paling banyak terganggu adalah pada fungsi atensi (p=0,020). variabel lain yang memiliki nilai signifikan terhadap fungsi kognitif adalah adalah merokok, lamanya riwayat hipertensi, dan pendidikan. Kesimpulan: Riwayat menderita hipertensi yang lebih dari 5 tahun menyebabkan penurunan fungsi kognitif terutama pada fungsi atensi.
