Resume Objectif : Presentation d’un cas de syndrome myelodysplasique traite a l’aide de la decitabine. Resume du cas : Il s’agit d’une femme de 80 ans, dont l’etat se deteriore rapidement apres un diagnostic de syndrome myelodysplasique. En quelques mois, l’anemie causee par la myelodysplasie devient tres incommodante, refractaire a l’erythropoietine et necessite des transfusions. L’equipe traitante etudie alors plusieurs options, puis choisit le traitement a la decitabine. La patiente recoit deux cycles de decitabine a raison de 15 mg/m2, toutes les huit heures pendant trois jours consecutifs, ces cycles etant espaces de six semaines. La patiente tolere tres bien le traitement et ne presente aucun effet indesirable. Discussion : Les syndromes myelodysplasiques constituent un groupe heterogene de desordres oligoclonaux de la moelle osseuse, se traduisant par une deficience progressive de l’hematopoiese et des cytopenies, des desordres qualitatifs des cellules sanguines et une propension a la transformation en leucemie myeloide aigue. Selon le pronostic des medecins, plusieurs options therapeutiques peuvent etre envisagees, telles que les soins de soutien, la greffe de moelle osseuse allogenique et la chimiotherapie. Plusieurs molecules se sont ajoutees recemment a l’arsenal therapeutique, et la decitabine, un inhibiteur de l’ADN methyltransferase, en fait partie. Conclusion : La decitabine est une option therapeutique interessante pour les syndromes myelodysplasiques symptomatiques ou a mauvais pronostic. Cette molecule est appelee a prendre une place de plus en plus importante pour le traitement de certains patients souffrant de myelodysplasie. Abstract Objective: To present a case of myelodysplastic syndrome treated with decitabine. Case summary: A woman 80 years of age presents with rapidly deteriorating status following a diagnosis of myelodysplastic syndrome. In a few months, the anemia caused by myelodysplasia becomes problematic and is refractory to erythropoietin and requires transfusions. Several options are evaluated by the treating team, and treatment with decitabine is chosen. The patient receives two cycles of decitabine dosed at 15 mg/m2, every 8 hours for 3 consecutive days every 6 weeks. The patient tolerates the treatment well and does not suffer any side effects. Discussion: Myelodysplastic syndromes are a heterogenous group of clonal myeloid stem cell disorders characterized by cytopenias and a progressive deficit in hematopoiesis, qualitative disorders of blood cells, and a propensity for transformation into acute myeloid leukemia. According to prognosis, many therapeutic options are possible including supportive care, allogeneic bone marrow transplant, and chemotherapy. Several new drugs have recently been added to the therapeutic arsenal, including decitabine, which is an inhibitor of DNA methyltransferase. Conclusion: Decitabine is an interesting therapeutic option for symptomatic or poor-prognosis myelodysplastic syndrome. This drug will likely become more important in the treatment of certain patients suffering from myelodysplasia. Key words: Myelodysplastic syndrome; decitabine; 5-azacytidine; lenalidomide
