Parsons a mené deux analyses parallèles du système d'action qu'est le rapport médecin/patient. La plus connue, celle qui a donné lieu à des études psychosociales empiriques, enumere les orientations normatives des rôles de patient et de médecin. La moins connue étudie le rapport clinique lui-même, plutôt que les orientations normatives des acteurs, qui le constituent selon le schéma ÁGIL. Parsons n'a pas abandonné l'une de ces analyses au profit de l'autre au cours de sa longue carrière. Lorsque sa définition des rôles de patient et de médecin est attaquée, il ne renvoie pas le critique à ses travaux les plus récents, comme dans plusieurs de ses réponses à ses nombreux détracteurs, il préfère défendre ses propositions premières. Dans ce texte, nous cherchons à unir dans un seul ensemble conceptuel les analyses parsoniennes des rôles de médecin et de patient et du rapport clinique. Puisque les orientations normatives de ces rôles sont définies par les variables structurelles, tandis que le rapport clinique est analysé selon le schéma ÁGIL, deux grandes périodes du développement de la pensée de Parsons sont invoquées. Leur unité passe cependant par une conception unique de l'action sociale comme une dynamique que l'on retrouve à l'origine des travaux théoriques de Parsons.
