Abstract

Venous thromboembolism (VTE) remains a significant cause of maternal morbidity and mortality in developed countries. The management of pregnant patients who develop deep vein thrombosis (DVT) or pulmonary embolism (PE) includes accurate diagnosis followed by appropriate treatment. Current guidelines surrounding these areas are still largely extrapolated from studies in the non-pregnant population. In the last decade, there has also been extensive literature suggesting an association between inherited thrombophilias and adverse pregnancy outcomes – appropriate clinical trials in this area are, however, lacking. This chapter updates the approach to the diagnosis of DVT and PE in pregnant women, and reviews the appropriate treatment of women with acute VTE during pregnancy. It also reviews the use of thromboprophylaxis in those who have previous VTEs and those who have asymptomatic thrombophila during pregnancy.
