The Role of Computer Navigation in Total Hip Arthroplasty

Abstract
Demand for Total hip replacement (THR) is rising steadily as a result of an ageing population and extended indications for surgery in the younger patient. Poor acetabular alignment increases the risk of dislocation and wear of THR. From a review of the current literature the best quality evidence in the form of randomized trials, meta-analysis and comparative trials have been carefully reviewed. Hip navigation reduces the outliers of acetabular positioning and can be performed either with pre-operative CT, or using imageless software. Cost effectiveness and clinical benefit have not been firmly established. Navigation of femoral head resurfacing appears effective but there is a lack of prospective randomised trials to confirm this.
