Abstract
I congratulate the authors for the effort of reviewing their experience on this topic. Treatment of vesicoureteric reflux associated with low bladder compliance is known to be challenging and to have lower success rates. This study reviewed a single center experience on endoscopic correction of vesicoureteric reflux using several different injectable agents, mainly polydimethylsiloxane. The authors compared patients with decreased bladder compliance as a result of several etiological causes (neurogenic, radiation, tuberculosis, pelvic surgery, etc.) with patients with normal bladder compliance. They found 70% cure rates in both groups of patients with a single endoscopic injection and with no remarkable complications. Despite the enthusiasm of the authors who conclude that this study ‘confirms the effectiveness of ESI in this population’, I would take this conclusion with a bit of caution. This study analyzed just 14 patients with decreased bladder compliance and found an acceptable cure rate, but the follow up is short and the number of patients is very small. Although interesting, we should use these results for hypothesis generation and grounds for further research, but strong conclusions are still precocious.
