En aquesta tesi sintentara contrastar la hipotesi de que la resposta clinica a un antipsicotic antagonista dopamina-serotonina esta relacionada amb una alteracio de la funcio serotoninergica a nivell cerebral. Per a avaluar in vivo aquesta possible disfuncio, es fa servir una estrategia neuroendocrina: ladministracio de lagonista preferencial del receptor serotoninergic 5-HT2C, m-clorofenilpiperacina (mCPP), provoca lalliberament de prolactina i altres hormones per part de la hipofisi, efecte que pot ser mesurat i bloquejat per antagonistes de la serotonina. Amb aquest objectiu es realitza un disseny prospectiu amb dades aparellades per al que es reclutaren 20 subjectes amb el diagnostic d'esquizofrenia (DSM-IV) en fase dexacerbacio aguda i lliures de medicacio antipsicotica en les dues setmanes previes a lestudi. Sels realitza una prova destimulacio neuroendocrina amb mCPP (05 mg/Kg) a nivell basal i a les sis setmanes de tractament a dosis estandard de risperidona. Es varen realitzar avaluacions cliniques (PANSS, CGI) i defectes adversos (Escala de Simpson-Angus, BARS, Calgary,...) i com a variables a estudi es defini duna banda lincrement hormonal (?PRL, ?GH, ?ACTH) com la diferencia entre la maxima resposta hormonal provocada pel mCPP i el nivell hormonal basal i de laltra la resposta clinica com el percentatge de reduccio en els valors de lescala PANSS total a les 6 setmanes de tractament. Com a conclusions es destaca que durant el tractament de les exacerbacions agudes de lesquizofrenia amb un antipsicotic amb activitat antagonista dopamina-serotonina, com es la risperidona, es produeix un increment marcat dels nivells plasmatics basals de PRL. Aiximateix, ladministracio de mCPP provoca una augment significatiu de les hormones hipofisaries en aquests pacients. Es remarca que un elevat alliberament hormonal de PRL induit pel mCPP (funcio serotoninergica) abans del tractament amb risperidona es correlaciona amb una reduccio dels simptomes positius en les exacerbacions de lesquizofrenia. Finalment i en contra de lesperat, el tractament amb risperidona no bloqueja la resposta dalliberament hormonal induida pel mCPP, suggerint a les dosis cliniques utilitzades no es comporta com un potent antagonista 5-HT2C.
