RÉSUMÉ: Malgré la reconnaissance de l'importance de l'allaitement maternel, seulement la moitié des mères québécoises pratiquent l'allaitement total durant les six premiers mois alors que moins de 20 % des mères pratiquent l'allaitement exclusif (Statistique Canada, 2010). Une meilleure compréhension des facteurs influençant la poursuite de l'allaitement s'impose afin de permettre de développer des mesures favorisant sa poursuite. Au cours des dernières années, on tend de plus en plus à percevoir l'allaitement comme étant un processus interactionnel privilégié entre la mère et son enfant, ce qui a un impact sur les recherches effectuées pour identifier les facteurs liés à la poursuite de l'allaitement. En effet, une telle façon de percevoir l'allaitement amène les chercheurs à examiner les facteurs pouvant influencer ce processus interactionnel. Dans ce sens, la présente étude vise à comparer la sensibilité maternelle et l'anxiété des mères qui poursuivent l'allaitement jusqu'à six mois à celles qui ont cessé. L'étude de type descriptive corrélationnelle a été effectuée à partir des données recueillies dans le cadre d'une étude primaire, effectuée par Lacombe, Bell et Lacharité (2011). L'échantillon est composé de 106 dyades mère-enfant en bonne santé dont les mères sont des primipares allaitant à la sortie de l'hôpital. La collecte des données s'est déroulée à 48-72 heures de vie de l'enfant ainsi qu'à deux et six mois. À chaque temps de mesure, les mères ont rempli un questionnaire autoadministré comprenant l'Échelle de sensibilité maternelle (Bell, Goulet, St-Cyr Tribble, et Paul, 2004) et l'Inventaire d'anxiété situationnelle (forme Y) (Spielberger, 1983). Afin de répondre au but de l'étude, des analyses descriptives et bivariées ainsi qu'une analyse de régression logistique ont été effectuées. Les résultats obtenus suggèrent un lien significatif entre la sensibilité maternelle dans les composantes de la découverte (1,5(1,0-1,9) vs 1,7(1,3-2,0); p=0,03) et de l'engagement (1,1 (1,0-1 ,5) vs 1,4(1,0-1 ,7); p=0,03) à deux mois ainsi que la proximité physique à six mois (1,4(1,0-1,7) vs 1,5(1,2-2,0); p=0,04) chez les mères qui allaitent jusqu'à six mois comparativement à celles qui ont cessé d'allaiter avant six mois. L'anxiété de la mère à 48 heures est associée significativement à la poursuite de l'allaitement à six mois (34,00(29,00-39,00) vs 30,00(26,00-34,00); p=0,04). De plus, la composante de la découverte (p=0,002) de même que les cours prénataux (p=0,03) et le revenu (p=0,003) ont été identifiés comme étant des facteurs prédictifs de la poursuite de l'allaitement à six mois. Les résultats de la présente étude démontrent l'importance des approches de soutien à l'allaitement qui prennent en compte l'aspect relationnel de l'allaitement. -- Mots clés : allaitement maternel, anxiété de la mère, sensibilité maternelle, relation mère-enfant, caring. -- ABSTRACT: Despite the fact that exclusive breastfeeding up to six months is highly important, less than 20% of Quebec mothers practice it as recommended (Statistics Canada, 2010). There is a need for a more comprehensive understanding of the factors that influence the continuation of breastfeeding. In recent years, the trend has been to think of breastfeeding as a privileged, interactional process between mother and infant, which has motivated researchers to attempt to identify factors that have bearing on this interactional process. This study aims to describe the relationship between mother-sensitivity and anxiety on breastfeeding continuation up until six months. This correlational, descriptive study was conducted using data from a prior study proposed in 2011 by Lacombe, Bell and Lacharité. The experiment involved 106 healthy breastfeeding mother-baby dyads. Data was collected 48 to 72 hours following birth, at two and six months. Mothers completed L'Échelle de sensibilité maternelle (Bell, Goulet, St-Cyr Tribble, et Paul, 2004), the State-Trait Anxiety Inventory (form Y) (Spielberger, 1983) and the Brown's Support Behaviors Inventory (1986). Sociodemographic, clinical and breastfeeding information were also collected. Data was analysed using descriptive and bivariate analyses as weil as a regression analysis. Results suggest that discovery (1 ,5( 1,0-1,9) vs 1,7(1,3-2,0); p=0,03) and commitment at two months (1,1(1,0-1,5) vs 1,4(1,0-1 ,7); p=O,03) as well as physical proximity at six months (1,4(1,0-1,7) vs 1,5(1,2-2,0); p=0,04) are reJated to breastfeeding up to six months. Anxiety at 48 hours was related to breastfeeding up to six months (34,00(29,00-39,00) vs 30,00(26,00-34,00; p=0,04). Discovery (p=0,002), prenatal class (p=0,03) and income (p=0,003) were identified as predictive factors of breastfeeding continued at six months. Results of this study highlights the need for breastfeeding approaches that consider maternal sensitivity as part of the breastfeeding experience. -- Keywords : breastfeeding, maternal sensitivity, maternal anxiety, mother-infant relationship, caring.
