Abstract
Hypertonie ist der bedeutendste singuläre Risikofaktor für die Gesamtsterblichkeit, weltweit verantwortlich für 7,5 Millionen Todesfälle jährlich. Zahlreiche Erhebungen der letzten Jahre zeigen im europäischen Raum Kontrollraten, d.h. den Anteil bereits diagnostizierter und therapierter Patienten, die das Blutdruckziel erreichen, zwischen 35 und 50 %. Im Vergleich gelang es in Kanada durch ein spezifischen Disease Management Program (DMP), dem Canadian Hypertension Education Program, Kontrollraten von 13 % in 1992 auf 67 % in 2009 anzuheben. Somit ist Kanada globaler Spitzenreiter. Von einer europäischen Expertenrunde wurden folglich Ziele und Ansätze zur Optimierung der Gesamtsituation formuliert. Darunter fallen eine Verbesserung der Primärprävention (körperliche Aktivität, Ernährung), Überwindung der therapeutischen Trägheit und somit konsequente Senkung bis zur Erreichung des Blutdruckziels, Vereinfachung der Therapie u. A. unter Verwendung von Kombinationspräparaten, Miteinbeziehung der Betroffenen (Stichwort Selbsttitration) und enge Zusammenarbeit mit Gesundheitsträgern. Diese Empfehlungen finden womöglich Eingang in nationale DMPs.
