Abstract
목적: 최근 첨단 Flat Panel 혈관조영기의 보편화와 함께 TACE 시술 시 정확한 병변의 진단을 위해 시행하는 C-arm CT의 유용성에 대해 CT 표준 Phantom을 이용한 영상평가와 리피오돌 흡수 영상 비교, 선량평가를 해 봄으로써 C- arm CT의 임상적 유용성을 평가 해보고자 하였다. 대상 및 방법: C-arm CT와 MDCT의 영상의 질을 비교하기 위해 AAPM (American Association of Physicists in Medicine) Phantom을 사용하였다. 선량측정 장비는 Mobile MOSFET System (Best medical Canada, Ottawa, Canada)을 Humanoid Phantom (Male ART, The Alderson Radiation Therapy) 내부에 장착하여 측정하였다. 결과: AAPM Phantom을 이용한 측정 결과는 MDCT가 전 평가항목에 대해 적합 판정을 받은 반면에 C-arm CT는 물의 CT감약계수, 균일도, 공간분해능, 인공물 검사 등에서 미달 판정을 받았다. MOSFET Detector를 이용한 피폭선량 측정 결과는 C-arm CT와 MDCT 모두 큰 차이가 없었고 통계 (Wilcoxon Signed Ranks Test)에서도 유의한 차이가 없었다 (p=0.686). 리피오돌 흡수영상 및 조영증가 영상평가에서는 간종양의 리피오돌 흡수부분이 주위의 간 실질부위보다 높은 밀도를 나타내 흡수 여부를 알아보는데 조금도 부족하지 않았다. 결론: 양질의 C-arm CT 영상을 얻기 위해서는 임상 영상의 평가 및 적절한 성능관리용 팬텀 및 프로그램에 의한 정도관리가 이루어져야 한다. 하지만, 현재 C-arm CT 장치에 대한 정확한 관리기준이나 검사수가가 마련되어 있지 않은 상태이므로 조속한 시일 내에 새로운 법적, 제도적 기준이 제시되어야하고 이에 따른 관리가 요구되어야 할 것이다.
