Presque tous les pharmaciens et pharmaciennes qui travaillent dans les hopitaux et les etablissements de sante canadiens exercent leur metier dans un environnement ou la liste des medicaments (couramment appelee le formulaire) dicte les medicaments qui sont disponibles pour traiter les patients. Le formulaire peut etre definit comme un recueil de medicaments continuellement mis a jour et qui est le reflet de l’opinion clinique actuelle du personnel de l’etablissement, sur les medicaments necessaires pour traiter la population de patients de l’etablissement. Le systeme du formulaire constitue la methode par laquelle les medecins, les pharmaciens et les autres professionnels
