Une transition fondamentale a lieu dans les hôpitaux à partir des dernières décennies du xix e siècle, alors que leur fréquentation s’élargit des groupes les plus pauvres de la société à l’ensemble de celle-ci. Effectuée à la faveur de la commercialisation et de la médicalisation des services offerts, cette transition est fort mal connue en ce qui a trait à l’évolution de la composition des populations hospitalisées. À l’aide d’une double étude de cas fondée sur les registres d’entrée et de sortie des patients, le présent article vise à préciser cette composition dans les années précédant la transition jusqu’à la Seconde Guerre mondiale, en termes d’âge, de genre, de provenances géographiques et de catégories socioprofessionnelles. Il en ressort, parmi divers constats, des bassins de recrutement étonnamment vastes, de nettes distinctions sociales quant à l’accès aux chambres privées et aux salles communes, des écarts parfois majeurs entre groupes d’âge et entre sexes.
