Abstract
Patients : Âges de 18 ans et plus, 12 heures a 10 jours post-infarctus du myocarde complique de signes cliniques ou radiologiques d’insuffisance cardiaque, d’evidence de dysfonction systolique du ventricule gauche (fraction d’ejection ≤ 0,35 a l’echographie ou a l’angiographie ou ≤ 0,40 a la ventriculographie), ou les deux. Leur tension arterielle systolique devait etre > 100 mm Hg et leur creatinine serique < 221 μmol/L. Les patients avec une intolerance anterieure ou une contre-indication a un inhibiteur de l’enzyme de conversion de l’angiotensine (IECA) ou a un antagoniste des recepteurs de l’angiotensine (ARA), une maladie valvulaire significative ou toute autre condition limitant significativement l’esperance de vie etaient exclus. Interventions : Les patients admissibles a l’etude etaient repartis aleatoirement dans les trois groupes suivants : 1) monotherapie avec le VAL; 2) combinaison VAL + CAPTO; 3) monotherapie avec le CAPTO. Le traitement etait amorce avec VAL 20 mg PO BID, VAL 20 mg PO BID plus CAPTO 6,25 mg PO TID ou CAPTO 6,25 mg PO TID. Les doses etaient augmentees graduellement en quatre etapes, avec le but d’atteindre l’etape 3 (VAL 80 mg BID, VAL 40 mg BID plus CAPTO 25 mg TID ou CAPTO 25 mg TID) pendant l’hospitalisation initiale, et l’etape 4 (VAL 160 mg BID, VAL 80 mg BID plus CAPTO 50 mg TID ou CAPTO 50 mg TID) si cliniquement possible, pour la visite du troisieme mois. Les investigateurs pouvaient ajuster les doses a leur discretion selon l’etat du patient. Six visites etaient prevues pendant la premiere annee d’inclusion et a tous les quatre mois par la suite. L’etude evaluait d’abord la superiorite du VAL et de la combinaison VAL + CAPTO par rapport a celle du CAPTO seul, mais advenant que cette analyse soit negative, les chercheurs avaient prevu une evaluation de non-inferiorite du VAL par rapport au CAPTO.
