Abstract
Résumé Le système de santé canadien se caractérise par la juxtaposition d’un panier de biens et services dont le financement est public (soins de médecins et hospitaliers), et pour lequel le financement privé est interdit ou très limité et d’un ensemble de consommations laissées pour une large part au privé (dentaire, médicament, soins à domicile, santé mentale, transports). Sur la partie publique, l’évaluation du besoin par les professionnels de santé est jugée plus légitime que la demande exprimée (au besoin en dollars) par les patients. Il en résulte évidemment un problème de listes et de délais d’attente pour l’accès aux soins de spécialistes et à l’hospitalisation programmée. Il en résulte aussi une certaine rigidité du système face aux changements de pratiques qui supposent l’inclusion de nouveaux types de soins dans le panier public (comme le virage ambulatoire). L’auteur décrit les principes du système, les tensions et débats qu’il suscite, et tente une comparaison avec le système français.
