Résumé Pour rester fidèles aux valeurs de la population qu’elles représentent, et préserver ainsi les fondements mêmes de leur légitimité, les démocraties modernes doivent infléchir la trajectoire actuelle de leur système de soins, car il répond de moins en moins bien aux attentes de la population. Par ailleurs, nous constatons que toutes les tentatives de réforme des systèmes de santé se heurtent depuis 30 ans à une inertie puissante. Dans la première partie, cet article analyse l’origine et la nature de cette résistance. Dans la deuxième partie, il explore certaines stratégies de transformation du système de soins pour le rendre cohérent par rapport aux attentes de la population. Le système de santé n’est ni une institution parmi d’autres, ni une industrie dont l’État pourrait se désintéresser; par conséquent, sa réforme est une responsabilité collective. Une approche participative et décentralisée est proposée en accord avec ces principes démocratiques fondamentaux. Cette approche s’oppose aux pressions en vue de l’uniformité de la part des instances en place, s’apparentant à un processus d’apprentissage continu qui mobilise la responsabilité des professionnels et leur permet de développer leur réflexion.
