Management of a Penetrating Gunshot Wound of the Trachea and Bullet Extraction Using Bronchoscopy

Abstract
A 34-year-old man presented with a gunshot wound fracturing the medial third of the left clavicle and penetrating the hemithorax. Chest x-ray demonstrated a metallic object suggestive of a bullet at the right hilar level. Computed tomography revealed bilateral subcutaneous emphysema, pneumomediastinum, and mild left pneumothorax. Initial exploration with flexible bronchoscopy revealed a left tracheal wall perforation in the superior third yet, no exiting orifice. The bullet location was confirmed to be at the right lower lobe bronchus. Considering that the diameter of the bullet was greater than that of the endotracheal tube, we extracted the bullet with a rigid bronchoscope. The tracheal repair was performed through a posterolateral thoracotomy. The patient recovered without any further complications. Combined use of flexible as well as rigid bronchoscopes was essential in the management of this patient.
